Family Health Centers At NYU Langone

The Family Health Centers (FHC) at NYU Langone, based in Sunset Park, is a federally qualified community-based ambulatory health care system (#FQHC) affiliated with Lutheran HealthCare. FHC at NYU Langone is recognized by the National Committee for Quality Assurance as a Level 3 Patient-Centered Medical Home. Governed by the NYU Lutheran Family Health Centers Board of Trustees, FHC is one of the largest health center networks in the country serving as southwest Brooklyn's medical home for more than 45 years. With nine full-time community health centers, 29 school-based health centers and community programs, including three WIC programs, adult and family literacy programs, three child care centers and a family wellness center, the network is the primary medical provider for more than 100,000 people providing nearly 700,000 patient visits per year.

COMMUNITY-BASED PROGRAMS

The Family health centers at NYU Langone Department of Community-Based Programs has a thirty-year history of providing educational and supportive services to community residents and their families. Our work is grounded in the philosophy that is the foundation of Lutheran's holistic orientation to community health - that health depends upon such factors as an individual's psychosocial, familial, economic, housing, educational, and employment status. We are committed to affirming each family's cultural, ethnic and linguistic identities and using community social work practices, work toward ensuring that our programs are always designed to be responsive to evolving family and community issues in the context of a diverse, multicultural society.
